This is a rare case of malrotation with proximal small bowel volvulus (whirlpool sign) & associated intestinal occlusion in an adult patient actually had intermittent abdominal pain for a few months, and the obstruction looks chronic / sub-obstructive since per os contrast migrated normally in the distal small bowel. Malrotation and volvulus should be suspected in all cases of proximal small-bowel obstruction, especially in infants. Besides the cardinal clinical manifestation of bilious vomiting, clinical features may include pain (colicky at first, then steady), anorexia, blood and mucus in the stool, abdominal tenderness, and, eventually, shock.
Intestine is that part of alimentary canal that extends from stomach to anus. It is divided to two parts i.e small intestine and large intestine. This division of intestine into two parts are due to some major differences between them. There are some similarities . We experenced five cases of comparatively rare primary volvulus of the small intestine, and all were completely cured. The patients comprised two girls aged 11 and 7 and three female neonates. The two girls could be cured by reduction of the intestinal volvulus, but intestinal resection was necessary in Author: Sumito Takagi, Muneyuki Yamaguchi, Setsuo Takeuchi, Hajime Kase, Hiroshi Nakamura, Kiyoshi Watanabe.
A diagnosis of volvulus of the entire small intestine was made during surgery in 25 cattle, whereas volvulus of the distal jejunum and ileum was diagnosed during surgery in 7 cattle. Survival rate following surgical correction of volvulus of the entire small intestine (44%) was not significantly different from survival rate following surgical. Volvulus. Ladd's procedure is the treatment of choice in most cases. Volvulus is corrected by rotating the small intestine in an anticlockwise direction, the caecum being placed in the left abdomen and the duodenum directed down the right paravertebral : Dr Colin Tidy.

A volvulus is when a loop of intestine twists around itself and the mesentery that supports it, resulting in a bowel obstruction.
Symptoms include abdominal pain, abdominal bloating, vomiting, constipation, and bloody stool. Onset of symptoms may be rapid or more gradual.
The mesentery may become so tightly twisted that blood flow to part of the intestine is cut off, resulting in ischemic cations: Ischemic bowel. Small bowel volvulus accounted for per cent of cases of all intestinal obstruction and per cent of all cases of intestinal volvulus.
It was more common than was volvulus of the sigmoid : Muneera Kapadia. Case Discussion. Classic case of small bowel volvulus. At surgery, two adhesive bands were felt responsible for the volvulus (from the op-note): "Midportion of the ileum was adhered deeply to the posterior wall of the abdomen which was causing a volvulus.
whenever possible. In five of the 12 cases, the volvulus was treated successfully and these patients survived. This indicates that the prognosis for mesenteric volvulus might be better than is currently believed, and immediate laparotomy is recommended if mesenteric volvulus is suspected.
Mesenteric volvulus in the dog: a retrospective study of. Volvulus of the small intestine is a condition of bowel obstruction due to knotting and twisting of the small intestine.
Two types of volvulus are described: 1) primary small intestinal volvulus where no predisposing factors exist, and 2) secondary volvulus where congenital or acquired conditions promote twisting of Author: Ivanov A, Viiachki I, Iarŭmov N, Viiachki D, Gerzilov P, Parunev M.
Depending on the location of the volvulus, symptoms may vary. For example, in patients with a cecal volvulus, the predominant symptoms may be those of a small bowel obstruction (nausea, vomiting and lack of stool or flatus), because the obstructing point is close to the ileocecal valve and small intestine.
In dogs, it is an uncommon condition, with German shepherd dogs showing a predisposition. Chronic mesenteric volvulus has also been described. In cats, previous reports have documented two cases of small intestinal volvulus, both diagnosed at necropsy, and a further case of volvulus of the colon in a patient that died after by: Small bowel volvulus (SBV) is defined as torsion of a loop of small bowel about the axis of its mesentery, resulting in partial or complete obstruction.
SBV is a rare cause of small bowel obstruction in Western countries, comprising 1–6% of cases [ 1 ].Cited by: 3. In cats, previous reports have documented two cases of small intestinal volvulus, both diagnosed at necropsy, and a further case of volvulus of the colon in a patient that died after surgery.
The small intestine with the exception of the duodenum, and the large intestine with the exception of the rectum, may become involved, although volvulus is most fre- quently found in the large bowel. The common sites are: the sigmoid colon, about 75 per cent of all cases are found here; the cecum; the cecum and ascending colon; the cecum Cited by: 4.
flap. Of 10 cases, nine (excluding two unknown cases) presented with the whirl sign on CT, making preopera-tive diagnosis possible.
When we examined the cause of the torsion in the 29 cases, many cases (n=13) had organic anomalies such as small bowel tumors or small bowel intussusception prior to the torsion, and these cases were classified asCited by: 1. If midgut volvulus is present, the entire small intestine along with the transverse colon is delivered out of the abdominal incision, where the volvulus can be reduced.
Because the volvulus. Small bowel volvulus is a rare clinical entity which presents as recurrent intermittent abdominal pain after consumption of food. Although the entity is well described in the literature, diagnosis is often difficult due to its clinical presentation being similar to mesenteric ischemia.
Herein we present the case of a year-old male who presented to us with this : Patial T, Chaddha S, Rathore N, Thakur. volvulus or bands.1 Upper Gastrointestinal Series An upper gastrointestinal (UGI) series is the preferred test for radio-graphic diagnosis of malrotation and volvulus (Figures and ).
It is usually performed with barium, except in cases of a very sick infant or child in whom the presence of infarcted bowel or perforation is pos.
Intussusception is categorized into two discrete categories: small intestinal and colonic. Computed tomography (CT) is the most accurate diagnostic modality for intussusception.
In adults, the pathologic lead point is a gastrointestinal (GI) malignancy in up to 56% of cases. With this in mind, the treatment is resection without : Katerina Wells.
Gastrointestinal volvulus is mainly classified into two subtypes, mesentero-axial volvulus and organo-axial volvulus. The detailed imaging findings of organo-axial volvulus of the small intestine have never been reported as far as we know. In this article, we report a case of organo-axial volvulus of the small intestine, focusing on the computed tomography (CT) : Toshitaka Ishiguro, Takashi Hiyama, Katsuhiro Nasu, Yoshimasa Akashi, Manabu Minami.
We report case of primary volvulus of the small intestine and review 15 cases from the Japanese literature. A year-old woman, with a history of appendectomy 30 years previously, was admitted with abdominal distension and signs of peritonitis.
Abdominal computed tomography (CT) demonstrated a whirl-like pattern of the mesentery, showing the tightly twisted mesentery around the Cited by: Introduction. A small intestinal (SI) volvulus occurs in the horse when the intestine rotates on its mesenteric axis through an angle greater than o, resulting in pathogenesis is thought to be due to a segment of hypermotile intestine preceding a segment where peristalsis has ceased, leading to the development of a twist.
volvulus: [ vol´vu-lus ] (L.) torsion of a loop of intestine, causing obstruction with or without strangulation. Volvulus. From Chabner. A 2 months-old girl presented with history of bilious vomiting, sonography showed signs of volvulus and emergency laparotomy was performed and confirmed left paraduodenal hernia containing a part of the ileon, coecum with right colon and volvulus of the .
Her unremitting abdominal pain, persistent vomiting, and absolute neutrophilia were consistent with an acute process. Small bowel (small intestine) - Volvulus. This website is intended for pathologists and laboratory personnel, who understand that medical information is imperfect and must be interpreted using reasonable medical judgment. In testing for volvulus, the following tests may be performed: A stool sample test finds blood in the stool.
An upper GI X-ray with small bowel follow-through shows a malrotated bowel or midgut volvulus. A CT scan may show evidence of intestinal obstruction. A barium enema often shows an abnormal position of the bowel, suggesting malrotation.
